After you applied the "Custom_Contact_Form_12x" profile:
- Visit the page Reply to validate the call to plugin Mail, which will allow you to reply messages from there
- Edit the newly created page Contact us
- Go to tab "Properties", and set "Show page title": Off.
- Visit as anonymous the page Contact us, and add a test message with a valid email.
- Visit as user admin the page Contact us in order to test the reply system
- Edit the tracker fields to suite your needs for the contact form
- Go to watch the newly created tracker
- Add a link to your Contact us page in your Menus (read the docs)
